FitCam Health’s Commitment to Your Privacy

Prove Labs Inc. (“Prove Labs”) provides the FitCam Health App to assist you in managing pain through wellness and telehealth services. Prove Labs and its healthcare providers are committed to protecting the privacy of your medical information. This HIPAA Notice of Privacy Practices (“Notice”) describes how we use and disclose your medical information and your rights related to your medical information.

 

Our Responsibilities

We are required by law to maintain the privacy and security of your Protected Health Information (PHI) and provide you with this Notice.   • Treatment: To provide Services to you and disclose PHI to your care team for treatment.

  • Healthcare Operations: To run our business, improve Services, train staff, conduct quality assessments, and contact you about Services. This includes de-identified or aggregated data.

We may also disclose your PHI under certain circumstances such as public health activities, protecting victims of abuse, judicial proceedings, law enforcement purposes, and other legally required instances.

We use electronic software, services, and equipment, including email, video conferencing, cloud storage, and internet communication, to collect and share your PHI. Information We Collect

  1. Personal Information: Includes your name, email address, date of birth, gender, pictures, and other details you provide when creating an account and using the app. If you register using a social media account, we collect information that the platform shares with us.

  2. Other Information: Automatically collected data includes IP addresses, device type, operating system, browser type, usage data, and information from messages, audio and video conferencing. We also collect data regarding your activities, such as steps from Apple Health or Google Fit, workout performance, and chat history.

 

How Your Information is Used and Shared

We use your information to provide and improve our services, communicate with you, and for ad optimization. We may share your data with service providers who assist us with hosting, customer service, and marketing, under strict confidentiality agreements. If invited by a healthcare provider, they will have access to your activity data to tailor your treatment plan.

In an ongoing effort to better understand our users, we may analyze certain Information in anonymized and aggregate form to operate, maintain, manage, and improve our services. This aggregate information does not identify you personally. We may share and/or license this aggregate data to our affiliates, agents, business and promotional partners, and other third parties.

 

Third-Party Communications Services
To enhance communication with you regarding your healthcare and services, we use third-party service providers, to send text messages and make phone calls. By using our services, you acknowledge and agree that we may share your phone number and related contact information with these third parties strictly for the purpose of facilitating communications, such as appointment reminders, service updates, or health-related inquiries. These providers are required to comply with HIPAA and other applicable privacy laws to protect your information. Changes to This Privacy Policy

This Privacy Policy is effective as of January 2019. We may update this policy periodically. Changes will be posted on the app. By continuing to use the app after changes are posted, you agree to the revised policy.
