We’re thrilled to have you join the FitCam My Care Coach™ team — a growing community of dedicated Nurse Coaches helping patients manage chronic pain through Remote Monitoring.

At FitCam, we believe that consistent, compassionate support between clinic visits can transform lives. That’s why we combine personalized nurse coaching, easy-to-use digital tools, and expert-designed programs like the Relief Journey to empower patients in their healing process — one conversation, one check-in, one small win at a time.

1. Understanding the Context: Two Ways We Support Pain Patients

At FitCam My Care Coach, our mission is simple: help patients living with chronic pain feel supported, guided, and empowered between visits. Many of these patients are struggling with long-term pain, often while taking opioid medications. They’re navigating not only physical challenges, but also emotional strain, isolation, and uncertainty about what’s next. That’s where you come in — not just as a nurse, but as a care coach and compassionate ally. Phone-Based Nurse Coaching (CCM) Some patients benefit most from simple, personal support — a real human checking in regularly by phone. A short conversation can help reinforce treatment goals, offer reassurance, identify concerns early, and make the patient feel genuinely cared for. This model works especially well for patients who aren’t comfortable with technology or who simply prefer to talk. At FitCam, we support this kind of care through structured monthly coaching and coordination, following the Chronic Care Management (CCM) model. It’s straightforward, meaningful work — and for many patients, it’s exactly what they need to stay on track. App-Based Digital Coaching (RTM) Other patients are ready to take a more active role in their recovery using digital tools. For them, we offer the FitCam App, a medical-grade platform that guides patients through self-monitoring and self-care. With the app, they can log their pain, movement, sleep, and mood. They also gain access to expert-designed content like personalized home exercise programs, guided relaxation, sleep support, and our holistic Relief Journey for mind-body healing. The app keeps them connected to you — their Nurse Coach — offering encouragement, structure, and accountability. This enhanced level of engagement is supported through Remote Therapeutic Monitoring (RTM). It’s especially powerful for patients motivated to take control of their progress and reduce reliance on opioids. At FitCam, we believe patients deserve both options — and many benefit from a combination. Whether you’re supporting someone by phone, through the app, or with both, your presence as a Nurse Coach is what brings the care to life.

2.1 Your Tools : Setting up Microsoft Email, Teams, Calendar 

Your FitCam Microsoft Account is your professional hub. It gives you secure access to your email, calendar, Teams for messaging, and patient booking tools. This account ensures all communication is encrypted and compliant with healthcare privacy regulations. Please complete the following steps before your live training session: Log in to Microsoft 365 using any browser: https://www.microsoft.com/en-us/microsoft-365/outlook/log-in Username: XX.YYY@fit.cam Temporary Password: R*IOjahfioaxxxx Download the Microsoft Authenticator App You will be prompted to use this app during your first login to verify your identity. Follow the on-screen instructions to set it up. Send a Test Email Once logged in, please send a quick test email to confirm your account is active and functioning properly. Install Microsoft Teams Download the Teams app (desktop or mobile) and sign in with your FitCam credentials. Once inside, please send a chat message to your onboarding contact so we can confirm everything is set up.

2.2 Your Tools : FitCam Console Account

Once your FitCam Console account is created, you’ll receive an email invitation to set up your password. After that, you can log in to the FitCam Console. When you access your account, you’ll find all the essential features at your fingertips. Under My Patients, you’ll see a list of the patients you’ll be working with. From there, you can start coaching and accessing their personalized dashboard. You’ll also be able to view key patient details, like phone numbers and emails, and add programs to support their care.

2.3 Your Tools : Dialpad 

Once your FitCam Console account is created, you’ll receive an email invitation to set up your password. After that, you can log in to the FitCam Console. When you access your account, you’ll find all the essential features at your fingertips. Under My Patients, you’ll see a list of the patients you’ll be working with. From there, you can start coaching and accessing their personalized dashboard. You’ll also be able to view key patient details, like phone numbers and emails, and add programs to support their care.

3. The FitCam Patient App

THe FitCam App is the Patient facing digital tool for your patients to access our Digital Pain Managment programs.

Please ask your Manager for a test invitation to make yourself familiar with the App and the content as you will have to help patient navigate it, try the different programs.
